Surgical Rhinoplasty Should the appearance of your nose be a huge cause for concern for yourself, you may well have investigated the prospect of surgical rhinoplasty. Still the most common form of plastic surgery, this surgical procedure will permanently alter the appearance of your nose. However, a surgical rhinoplasty is not without its downsides. According to estimates, severe or life-threatening complications occur with a surgical rhinoplasty in 1.7-5% of all cases. You would be undergoing anaesthesia, which means you are put in a state of controlled unconsciousness by your surgeon. You will not undergo any anaesthesia and be put to sleep, while the results are only temporary. Regulation within the UK aesthetics industry currently It comes as a great surprise to many that the aesthetics industry in the UK is unregulated. This means there are to date no requirements for aesthetic practitioners to have undertaken any prior training before they start injecting. Many aesthetics “courses” will promise to have you trained to provide non-surgical treatments such as lip fillers, Botox and fat burning injections in just a few days. The practitioner will be rushed through the course, with little hands-on experience and a very surface level knowledge of anatomy, if any at all. As the aesthetics industry is unregulated, this means the practitioner will face no consequences for practicing with little to no training, legal or otherwise. As you can imagine, this is extremely dangerous for clients who seek treatments from these rogue practitioners, as they have no knowledge of how to identify or deal with complications, and little anatomical knowledge to ensure they are injecting safely. Without regulation, the client will have to seek alternative courses of redress should they receive botched treatment, or even worse, become severely injured. What do we know about aesthetic licenses currently? It was a huge relief to many practitioners within the aesthetics industry when the UK government announced in March 2022 that it would be introducing a new amendment to the Health and Care Act 2022 which would give the Secretary of State the power to introduce a licensing scheme for non-surgical cosmetic procedures. Under the proposed licensing scheme, all aesthetic practitioners will be required to obtain a license to carry out aesthetic treatments. This encompasses all treatments which penetrate beyond the dermis layer of the skin, which includes Botox, dermal fillers, PRP and vitamin injections. There will also be a legal requirement to obtain insurance. Practicing without a license will be a criminal offence and enforceable by Environmental Health. Though the new licensing proposals have been announced, they have not yet been passed into law. This entails a long procedure in which the bill must be debated in Parliament and passed in both the House of Commons and House of Lords, before receiving royal assent. Following the passing of the new law, there is also likely to be a grace period before the law is enforceable. We therefore have no definitive indication of a timeline before the new licensing measures come into play. Although it has not yet been decided what the licensing scheme will entail, it has been announced that practitioners will need to meet certain educational criteria to be eligible to obtain a license to practice. The educational standard required will likely need to be accredited, meaning it has been approved by a professional organisation and meets quality assurance standards. What will you have to do to ensure you can obtain the license? Though the precise criteria required to be met is yet to be announced, the aesthetics industry has speculated a level 7 certificate in aesthetic practice will potentially be required to obtain an aesthetics license. The level 7 Certificate in Aesthetic Practice is a globally recognised qualification equivalent to a post-graduate level degree. It is designed and regulated by the awarding body QUALIF, ensuring the qualification meets exceptionally high standards, and is based upon Health Education England guidance relevant to the delivery of non-surgical cosmetic interventions. The level 7 is open to both medics and non-medics, including aesthetic practitioners who have completed a level 5 Certificate in Aesthetic Practice, or professional qualified personnel with qualifications equivalent to a level 6 degree, who is registered with a national professional health care body. What does this mean for my existing training? At Second Look Aesthetics, we have been anticipating developments within the aesthetics industry for some time. The licensing scheme is likely not going to come into law for a while and we still do not know for certain that level 7 will be required. As such, at this stage we would advise you to ensure your current aesthetics training is of a high standard and will enable you to enrol in the level 7 Certificate in Aesthetic Practice, should it be required in the future. However, the level 7 Certificate in Aesthetic Practice is an excellent qualification which will not only ensure safe practice, but also provide you with the ability to deliver treatments to the highest standard. As you are likely already aware, any aesthetic treatment involves an element of downtime. The inflammatory reaction from the injection will result in minor, temporary side effects such as bruising and swelling. However, there are a number of steps which you can take to speed up the healing process, leaving you able to enjoy your results even quicker. Before receiving any dermal fillers or other cosmetic injectables, it's crucial to follow the recommended pre and post treatment instructions to aid in your recovery. Pre-treatment Advice: Make sure you are well hydrated and rested before your treatment. This will help the skin to be plump and hydrated before your appointment and assist in the healing process. Avoid drinking any alcohol for 24 hours before your appointment. Alcohol thins the blood which results in additional bleeding during injection. The more that you bleed, the more likely you are to bruise. For the reasons stated above, avoid any blood thinning medication for five days before your treatment, including ibuprofen and aspirin. Avoid strenuous exercise on the day of your appointment. Flushed skin can increase the risk of bruising due to the increased amount of blood cells at the skin’s surface. If you suffer with cold sores, begin a course of anti-viral medication 14 days prior to your appointment, due to the increased risk of them erupting at the injection site. Do not wear makeup on the area being treated to your appointment. This increases the risk of infection, so you will be asked to remove any makeup before the treatment can begin. Avoid waxing, bleaching, tweezing, using hair removal products on the area being treated. This will make the area more sensitive and so more painful during the treatment. Stop anti-aging skincare products containing Tretinoin (Retin-A, Steivaa), Retinol, Retinoid, Glycolic Acid, or other exfoliating agents. Again, these will increase sensitivity in the area. We recommend taking Arnica supplements or regularly eating fresh pineapple seven days prior to your appointment. Both contain Bromelain, which will lessen any bruising.   Post-treatment advice: Continue taking Arnica supplements or eating fresh pineapple, which will reduce any bruising you experience more quickly. Take an antihistamine to reduce swelling. Apply an ice pack to the area treated to reduce swelling. Ensure the ice is covered in a cloth and not directly placed onto skin. Avoid itching or massaging the area treated. Avoid high temperatures, including hot drinks, saunas, steam rooms and hot tubs. Intense heat can further increase swelling. Avoid applying makeup to the area treated for 24 hours post treatment, as this increases the risk of infection. Avoid drinking alcohol and taking blood thinning medication for 24 hours post treatment, as this will increase your risk of bruising. Botulinum Toxin, commonly referred to as “Botox”, is the most well-known aesthetic treatment worldwide. Celebrities have long been fans and devotees, making the treatment a household name since it first entered the market in 1989. It is a neurotoxin protein which targets the nervous system. When injected into a muscle, it causes it to become temporarily paralysed. Most commonly used in the upper half of the face, it is an excellent anti-ageing tool. When injected into the facial muscles responsible for expression, the subsequent paralysis of the muscles results in fine lines and wrinkles being prevented from forming or deepening. As a result, you are left with a smooth, youthful and refreshed appearance. Many clients are concerned that Botox injections will leave them looking unnatural and without any facial expression. However, in the hands of a skilled injector, Botox can minimise wrinkles while retaining movement in your face, for a very natural appearance. As Botox is used to temporarily paralyse muscles, it is also used in other areas of the body to achieve various results. This includes in the masseter muscle to prevent migraines and teeth grinding, in the armpits to prevent sweating by around 82-87% and in the superior lip muscle to correct a gummy smile, among others. When should you get Botox treatments? Botox is a preventative measure, not a treatment for deep-set wrinkles. Therefore, you should begin getting Botox treatments as soon as you notice fine lines and wrinkles while your face is in a resting state. This is usually around age 25, however varies between individuals, dependent on genetics and lifestyle. Someone more genetically inclined to wrinkles who also smokes and has experienced a high amount of sun exposure will find they need treatments earlier. Meanwhile, someone who is less genetically inclined to wrinkles, while also maintaining a good skincare routine and ensuring they use sun protection will be able to wait longer. Seeking Botox injections too late when you already have deep-set wrinkles will mean the treatment is ineffective in achieving the results you hope for. While Botox will improve their appearance to a degree, it cannot reverse wrinkles, but instead prevents them from forming. The results last around 4-5 months, depending on the individual. Someone with stronger facial muscles will find the results do not last as long, while someone with weaker facial muscles will be able to wait longer between treatments. In this context, a dissolving session isn’t to correct a mistake. It is to prevent migration from occurring or worsening, to make sure your lip shape looks the best that it possibly can. How do we dissolve lip filler? The removal a client's lip filler can be accomplished easily through lip filler dissolving. The procedure is similar to a lip filler treatment, and is quick, safe and effective. After cleaning, disinfecting, and numbing the area, the practitioner will inject an enzyme called Hyaluronidase. The treatment area will then be massaged to promote the breakdown of the filler.
